We have an excellent opportunity for the right person. We are Dialysis Center in Lompoc, California. We provide compassionate and professional care for our patients. We have a current opportunity for a newly licensed or experienced MSW, LCSW or ASW. In this role, under the direct supervision of the Clinic Nursing Manager, you will provide comprehensive clinical social care to patients and their families, collaborate with physicians and oher healthcare professionals and be a major contributor to the overall well-being of our patients and their families.
The responsibilities in this role include the following:
- Assess the nature of the patient's situation by interviewing and reviewing the personal history
- Develop and execute individual treatment plans
- Participate in Care Treatment plans to gauge the patient's progress and make appropriate suggestions
- Communicate patient updates to the physicians, nursing staff, and management team as needed
- Facilitate refererrals, admittance and discharges to other healthcare professionals and community resources and programs
- Maintain accurate patient documentation
Requirements of this role include the following:
- Knowledge of social work in a clinical setting
- Compassionate and caring demeanor
- Ability to build repport with patients, their families and other staff members
- Strong leadership qualities
- Excellent communication skills - (Bi-lingual English/Spanish) not required, but highly desirable
