Provides program coordination as well as leadership and management of nursing staff on site. Works with Site Medical Director, and Director of Operations to direct the delivery of medical services. Requires knowledge of policies, procedures and regulations of nursing programs, supervisory techniques, personnel policies and procedures. Will coordinate smooth clinic function and participate in the management of associated clinical programs. Represents nursing on a variety of committees. Exercises considerable independent judgement to select appropriate course of action as required in a variety of situations. Responsible for planning, preparation, utilization, evaluation and monitoring of budget.
Provides professional, patient centered nursing care in the ambulatory setting in accordance with the multi-disciplinary plan of care and established policies and procedures, under the supervision of the Leadership Team. Collects and analyzes patient data, assists with development and maintenance of care plans, and evaluates outcomes of interventions indicated by the multi-disciplinary care plan. Documents and communicates these interventions and outcomes per professional and organizational standards.

Strategic Leadership
- With Clinical Leadership Team, coordinates function of clinical areas and that of associated clinical programs.
- Develops and maintains RN/MA orientation and education curriculum.
Compliance, Policy, and Procedure
- Develops, supports and implements policies and procedures.
Communication
- Attends and represents nursing at site and organization-wide committees, leadership team meetings.
- Meets weekly with other site leadership, site RNs, and monthly with Lead RNs
Teamwork
- Works with other departments to assess need for, and, as necessary, develop workflows and programs to best deliver high-quality patient care with focus on meeting Quality metrics.
- Provides nursing support to all patients assigned including assessment, education, and follow up.
Performance Management
- Establishes standards of performance for on-site medical assistants
- Facilitates communication and active problem solving while serving as a resource and mentor to staff
- Supervises day to day activities of the clinic team through direct observation and works toward improving patient flow and cycle times.
- Demonstrates excellent interpersonal and team building skills.
- Manages schedule, clinical coverage, attendance at site, organizational meetings for nursing team
- Ensures that staff appropriately and professionally delivers quality patient care.
- Provides direction and support for clinical team in order to achieve clinical goals and organizational objectives
- Coordinates orientation and education of new staff
- Oversees compliance of staff to infection control policies
- Encourages and provides continuing educational opportunities for staff including off-site courses, conferences, and webinars.
- Works with clinical team to ensure accuracy and adherence to all daily, weekly and monthly monitoring of clinical logs and tasks.
- Assures documentation in the EHR is completed and accurate utilizing a peer review process.
- Delegates tasks to other licensed and unlicensed care providers as appropriate.

- Facilitates communication and active problem solving while serving as a resource and mentor to staff.
Problem Solving and Decision Making
- Utilizes assessment skills to evaluate patient needs and formulate a plan of care, as part of the multi-disciplinary care team. Revises outcomes and interventions based on results of ongoing evaluation, collaborating with both the patient and members of the care team.
- Identifies patients at risk for poor outcomes and those who may require more intensive services; provides additional outreach and frequent follow up (by phone and in-person) to this population.
- Provides Complex Case Management, including chronic disease case management, critical diagnosis care coordination, transition care management, high risk clinical tracking, complex medication management and system utilization to appropriate patients.
Patient Focus
- Facilitates and monitors for effectiveness of communication to patients and appropriate follow up is maintained as needed to assure patient needs are met.
- Assists with triage processes and addresses patient’s immediate needs with ability to recognize urgent/emergent situations.
Willingness to Learn
- Maintains membership in professional organizations such as the ANAC, AAACN.
- Requests coaching and assistance from preceptors/other experienced staff when needed.
Professionalism
- Consistently displays professionalism in dealing with customers, staff, and clinicians as observed by manager. Pitches in to cover shortages within departments.
- Demonstrates awareness of cultural, ethnic, gender and racial differences and their potential to impact patient outcomes.
Skills and Abilities:
- Ability to work with computers in word processing, and database application preferred.
- Must be flexible and adapt to a changing, fast-paced environment.
- Must attend five CNE in-services annually.
- Ability to working remotely as needed, laptop and remote access provided.
Education and/or Experience:
- Current Licensure by the Maryland Board of Nursing as a Registered Nurse
- Required: Bachelor's in Nursing (BSN).
- Desired: Masters in Nursing or related field.
- Required: Three years of nursing in an ambulatory or acute care setting.
- Significant related experience with populations served may substitute for above requirement.
- Desired: Five years of nursing in an ambulatory or acute care setting.
Working Conditions/Physical Demands:
