JOB SUMMARY: The Social Worker seeks to improve the well-being of individuals experiencing homelessness by providing direct social work and case management services, including the coordination of community resources. The position provides a variety of individual and/or group supportive services that address both short-term and long-term client needs.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Interview clients for direct service needs, develop a case plan with the client including written short-term and long-term goals and tasks. Make referrals as needed.
  • Conduct ongoing assessment of client needs and reevaluate goals with clients at regular intervals. Deliver and coordinate services to individuals and maintain direct contact.
  • Facilitate daily crisis management, acting as a liaison between client and client’s environment.
  • Collaborate with other disciplines including legal, medical, educational, vocational, substance abuse and mental health professionals, and other service providers.
  • Make appropriate referrals for emotional, legal, medical, dental, education, recreational, vocational, employment, and housing needs.
  • Create and maintain case records, including, but not limited to legal documentation, assessments, case notes, case plans, action steps, service updates, court reports, medical reports, referrals, discharge summaries, visitation records, and educational information.
  • Complete reports and maintain statistical data on an ongoing basis.
  • Engage in continuing education activity to maintain licensure.
  • Demonstrate commitment to improving client services outcomes and customer satisfaction.

EDUCATION and EXPERIENCE:

  • Master’s degree in Social Work. 
  • LGSW Social Work license from the appropriate jurisdiction.

SKILLS and COMPETENCIES: 

  • Ability to communicate effectively, both written and oral.
  • Ability to respond to and work effectively with stakeholders.
