Sustainable Resources Center, Inc. (SRC) is a Minneapolis non-profit that administers weatherization and healthy homes programs in Minnesota. In our 47-year history, we have helped tens of thousands of families by making their homes more safe, healthy, and energy efficient.

Lead poisoning in children continues to be a national crisis. The Community Health Educator (CHE) plays a critical role in the fight against this crisis.  SRC’s Education & Outreach program includes community-based lead poisoning screening and education, in-home visits, case management and referral for lead hazard control services, and the CHE is a client’s direct connection to our program and services. Community Health Educators represent SRC in public, on the phone, and during in-person visits at the client’s home.

You’d be a great Community Health Educator if you:

  • Can maintain oversight over your own workload and set priorities accordingly.
  • Have computer skills and are very comfortable with modern office technology such as a copier, a scanner, and Microsoft Office Suite and 365.
  • Have daily access to an automobile and a relatively clean driving record (more details below).
  • Have a high school diploma or equivalent.

 PRIMARY JOB DUTIES:

Case Management: CHEs work diligently to engage and serve clients and must be tenacious with outreach via a variety of modes (phone, text message, e-mail) to contact clients and move them forward within our program. During that process, thorough notetaking in the client database helps to keep the whole team aware of case progress and provides an outstanding paper trail if a client is unreachable or drops out of the program voluntarily.

In-home visits: At an in-home visit, a CHE will assess the client’s eligibility for a lead mitigation grant by checking for lead paint hazards, and will also assist the client with the grant application. The CHE will also identify ways the household could be safer and healthier and make recommendations for ways SRC could help, such as pest control and safety measures. 

Community Events: SRC is highly active in the community, attending events in our one-of-a-kind mobile blood lead testing van Leadie Eddie. CHEs participate in these outreach events by providing information and answering questions about lead poisoning prevention to eventgoers, and inviting kids and pregnant individuals for free blood lead testing, which is performed at our van via finger stick by a qualified medical professional. CHEs also provide support before and after these events by ensuring the van is clean and well-stocked, and all documentation is in order.

Community Education: SRC provides classes to the community on Healthy Homes assessment and Lead Handling and Removal processes. These classes are both in person and online. CHEs both assist with and lead these classes.

NECESSARY PHYSICAL FUNCTIONS:

  • Can lift 30 pounds occasionally and 10 pounds frequently.
  • Can see to identify lead and healthy homes hazards in homes.
  • Has the physical ability to navigate client homes which may include stairs and other uneven surfaces.
  • Has the manual dexterity to type on a keyboard and write with a pen.
  • Can hear and speak to communicate with clients and colleagues via phone and in person.
  • Is able to drive a vehicle.

PREFERRED QUALIFICATIONS:

  • Experience in community outreach, community education or Social Work.
  • Bachelor's degree in Social Work.
  • Prior case management experience.
  • Bi- or multilingual, particularly in Spanish, Hmong or Somali.

This is a full-time position eligible for paid time off, vacation, paid holidays, 401(k) with employer match, medical and dental insurance, employer-paid life insurance, short-term disability and long-term disability benefits, and a cell phone stipend.

Community Health Educators drive their own vehicles throughout the community to visit our clients. Applicants will need a valid driver’s license, a relatively clean driving record, and daily access to an insured automobile. Mileage is reimbursed at federal rates.
