IIH Qualified Professional (QP)
Intensive in-home service means family preservation interventions for children and
adolescents who have or are at-risk of serious emotional
disturbance, including individuals who also have a diagnosis of intellectual
disability. Intensive in-home service is usually time-limited and
is provided typically in the residence of an individual who is at risk of being moved to
out-of-home placement or who is being transitioned back home from an out-of-home
placement. The service includes 24-hour per day emergency response; crisis treatment;
individual and family counseling; life, parenting, and communication skills; and
case management and coordination with other services.

Job Responsibilities:
Licensed or Qualified Professional has responsibility for the following under the direction of
the Team Lead and Program Director:
To coordinate and/or oversee the initial and ongoing assessment activities, convening the
child and family team for person-centered planning
? Ensuring linkage to the most clinically appropriate and effective services including
arranging for psychological and psychiatric evaluations
? Providing and coordinating behavioral health services and other interventions for
the youth or other family members with other licensed professionals and Child and
Family Team members.
? Monitoring and documenting the status of the recipient’s progress and the
effectiveness of the strategies and interventions outlined in the Person Centered Plan
? Providing Clinical Supervision for Qualified Professional’s and Associate
Professional’s on the team
? Participating in the person-centered planning process
? Completing the initial development and ongoing revision of the Person Centered
Plan and ensuring its implementation (Qualified Professional)
? Assisting with implementing a home-based behavioral support plan with the youth
and his or her caregivers as indicated in the Person Centered Plan
? Providing psycho education as indicated in the Person Centered Plan Assisting the
team leader in monitoring and evaluating the effectiveness of interventions, as
evidenced by symptom reduction and progress toward goals identified in the Person
Centered Plan

? Assisting with crisis interventions
? Assisting the team leader in consulting with identified providers, engaging
community and natural supports, and including their input in the person-centered
planning process
? Provide “first responder” crisis response, as indicated in the Person Centered Plan,
24 hours a day, 7 days a week, 365 days a year to recipients of this service.
Major Responsibilities:
• Provide behavioral support to consumers and families within the community and
home environment.
• Work with team lead and other team members to coordinate schedules and
provide required visits for each consumer and family.
• Ensure case management services are provided for each consumer.
• Assess each beneficiary's needs for comprehensive services.
• Develop and implementing the PCP.
• Convene Child and Family Team meetings to coordinate the provision of multiple
services and the development of, and revisions to, the PCP.
• Link the beneficiary or family to needed services and support.
• Monitor the provision of services and supports. Assessing the outcomes of
services and supports.
• Collaborate with other medical and treatment providers.
Collaborate with other behavioral health service providers, as well as with identified medical
(including primary care and psychiatric) and non-medical providers (e.g., Dept. of Social
Services, school, Dept. of Juvenile Justice), engages community and natural supports, and
includes their input in the person-centered planning process.
Knowledge, Skills and Abilities: Ability to listen effectively; knowledge of child and
adolescent development; knowledge of factors which may contribute to family dysfunction;
knowledge of behavioral management techniques; general knowledge of community
resources and services; knowledge of crisis intervention strategies; good problem solving
skills; ability to maintain effective working relationship with other social services providers,
service purchasers, and clients; ability to communicate effectively both orally and in writing.
Educational and Experience Requirements:
ALL Staff providing Intensive In-Home Services to children and families must
have a MINIMUM of one (1) year documented experience with this population.
The Licensed or Qualified Professional must meet ONE of the following qualifications
based on the targeted population being served:
a) Currently licensed by the appropriate North Carolina licensure board as a Licensed
Clinical Addiction Specialist, Licensed Clinical Social Worker, Licensed Marriage and
Family Therapist, Licensed Professional Counselor, Psychiatrist, Licensed Psychologist or a
Licensed Psychological Associate OR
b) A graduate of a college or university with a Master’s degree in a human services field and
has one year of full-time, post-graduate degree accumulated MH/DD/SAS experience with
the population served or a substance abuse professional who has one year of full- time, post
graduate degree accumulated supervised experience in alcoholism and drug abuse
counseling OR

c) A graduate of a college or university with a bachelor’s degree in a human service field and
has two years of full-time, post bachelor accumulated MH/DD/SAS experience with the
population served or a substance abuse professional who has two years of full time, post
graduate degree accumulated supervised experience in alcoholism and drug abuse
counseling or a registered nurse who is licensed to practice in the State of North Carolina by
the State of North Carolina by the North Carolina Board of Nursing who also has four years
of full-time accumulated experience in MH/DD/SAS with the population served.
d) A graduate of a college or university with a bachelor’s degree in a field other that human
services and has four years of full time, post bachelor’s degree accumulated MH/DD/SAS
experience with the population served, or a substance abuse professional who has four years
of full-time, post-graduate degree accumulated supervised experience in alcoholism and
drug abuse counseling.
The Qualified Professional to recipient ratio shall not exceed 1:12 for each team.
The Qualified Professional must be available for monthly group and individual supervision
and other job-related trainings
Qualified Professionals must be able to deliver services in various environments, such as
homes, schools, court, secure juvenile detention centers and jails (for state funds only*),
homeless shelters, street locations, and other community settings
